I'm in the process of performing a similar upgrade.
Instead of doing a big upgrade I went with several incremental
upgrades between major releases.
It provides better control over the entire upgrade process.

speaking kamailio script is largely back compatible to 4.x, with more changes occurring in
module parameters than in the routing portion of the config. I'd recommend setting up
a 5.8 Kamailio server (along with any other necessary services like a DB) in docker
compose and trying your script as a basic sanity test. You can read the release notes for
each minor version (4.2, 4.3, 4.4, 5.0, etc) as the changes tend to be fairly well
documented.

I am considering to release Kamailio v5.8.4 (out of branch 5.8) sometime
next week, most likely on Tuesday, Nov 12, 2024. If anyone is aware of
issues not yet on the bug tracker, report them there asap in order to
have a better chance to be fixed.
