Should I need to set a some special environment variable to load ldap.so in
the /etc/init.d/kamailio?

Just in case, have you set perhaps any of the environment variables
manually for working with ldap? 'service' command is a wrapper that calls
env -i (start with an empty environment), maybe this is the reason it works
differently for the root and kamailio users.
