Update after a few more days in production.
This still seem to happen, but very seldom:

Nov 30 12:50:22 /usr/sbin/kamailio[25529]: WARNING: db_mysql [km_dbase.c:91]: db_mysql_submit_query(): mysql thread id changed due to reconnect in mysql_ping
Not sure if it would make sense to re-try the mysql_ping a few times?
