I can't seem to recreate it any more. I compiled from source with DBG_QM_MALLOC and it worked fine with no segfault and $avp(cnam) got populated from the http_query function.
So I completely removed kamailio including the libs, etc. and did a yum reinstall on it to get the original prebuilt version I was testing with and it also worked fine with no segfault.
