192.168.1.LAN is LAN ip running rtpproxy and kamailio 66.41.221.WAN is WAN ip with ports 5004-5082 forwarded to 192.168.1.LAN And firewall turned off.
maybe you can send the ngrep output on port 5060 for such call. The network details you sent are hard to follow and don't have all needed details.
