The problem is how do you know from whom the first
INVITE is coming from? Also, what if the first user
tries to make a second call?

you could use avpops,
when u get invite from first user, write to db, when
second invite comes
check if db field exists if so deny call,

I know that SER by it self doesn't include a
parameter or option to
limit the max concurrent calls for a sip user.
Some UA like Linksys PAP2 have the option to
configure it to provide
dial tone only if they are registered with the
Proxy you could try that
option.
