This is wrong. I already suggested you to compare your loose_route section with that in the Kamailio default config file, but it seems you haven't done it.
I hope you know that ACK after INVITE-200 is a *new* transaction, and an in-dialog request (so it goes into loose_route section). Question: is your loose_route section allowing this ACK to be routed? I just see the t_relay for INVITE, what about other requests as INFO, PRACK, NOTIFY... *ACK* ?
