You will need a media gateway, or someone to provide a gateway service for you. Configure SER to forward the calls (t_relay etc.) to the gateway for outbound calls, and configure the gateway to use SER as a proxy for inbound calls. There are configuration examples for the former shipped with ser. grep for t_relay.
