I'm Marco a colleague of Andrew.
When we restart kamailio all the counters are correctly set to 0.
If there are active calls in the system the active_dialogs counter is then set to the number of records (dialogs) saved in db.

the rpc is not being used, but we do have a dlg_timeout of 12 hours.
There are a few calls every day that are terminated on timeout, but this
is a very small number compared to the total amount of calls on that system.
