Do you have the pcap with the traffic of this call?
Does the ACK has the
To-tag? Do you access dialog in any other way before calling dlg_manage()
(e.g., by $dlg_var() or other function from dialog module)?

What I noticed strange here, is that SL module reporting:
* sl_filter_ACK(): too late to be a local ACK! *
I'm not sure that this is the core of the problem, but maybe you can spot
something here.

is a buggy UA messing RR because this is happening with 4 Providers
randomly.
What else I noticed is that this is happening mostly with ISP's using
authorization, with other ISPs using IP as whitelisted source I didn't
notice anything like that.
Could this be related somehow?
