Thank you for the answer . I can not set Public IP address because these asterisk boxes are in small customers with 5 to 6 extensions and they dont have public IP address . 

You have mentioned its possible but How can can it be done ? Any hint please ? 

Please pay attention my kamailio is not in the same network so Its not a NAT . 

I have asterisk boxes in small customers that they dont have public IP address and now I want to register softphones from outside to their PBX . I have a kamailio in the cloud with Public IP address .

There are plenty of reasons to front-end it with Kamailio, e.g. protecting/securing it. But you haven't articulated those reasons.
