Sounds to me as what you want to do require 'a
few'
code changes to
Asterisk. Maybe I am wrong, but this might take some
work to get right.

open(ser): outbound proxy with public ip port=5060

address
which one is forward to asterisk for
registration.
When user agent are registered in asterisk AOR is
sip:user@private ip so asterisk query
"sip:user@private" behind nat (not possible).
