one way to do it is to add a "nated" parameter to record-route header
and then check it during mid-dialog request processing. that would tell
you that at least one of the UAs is behind nat.
on the other hand, if you use mediaproxy, the new version does not
create a new media session if there was none for the dialog earlier.
