If the client does not support STUN (or is behind a symmetric NAT) you also have to use the nat traversal function from the nathelper module (fix_nated_register, fix_nated_contact) and enforce an RTP proxy (force_rtpproxy() for rtpproxy or use_media_proxy() in case of mediaproxy)
