Note the openser, this tries to lauch openser, but since it is already running it will not bind to any interface and kill itself, during that process I do believe it does clean up behind it self and "correctly" it does delete the /tmp/openser_fifo file.
So I would consider this as most errors a human error..

Similar issue was reported some time ago and fixed in trunk with rev5093 and 1.4 with rev5094 by mid of October. http://openser.svn.sourceforge.net/viewvc/openser?view=rev&revision=5094
Are you using 1.3.x? I can backport to that branch.

At times when I want to get the current user list using /usr/local/sbin/openserctl ul show I get this error

*ERROR: Error opening OpenSER's FIFO /tmp/openser_fifo*
*ERROR: Make sure you have the line 'modparam("mi_fifo", "fifo_name", "/tmp/openser_fifo")' in your config*
ERROR: and also have loaded the mi_fifo module.*
*I dont think this is a configuration error, as killing and restarting openser does fix this. *
