your asterisk box needs to send a publish message to kamailio whenever status of mailbox changes. see kamailio presence_mwi module.
i don't know anything about asterisk,
asterisk should responde with notify to subscribes for mwi, so just divert the SUBSCRIBE requests for message-summary to asterisk.
