the problem might be in iptel configuration -- regarding the policy of
accepting the requests. I will try to reproduce the case it but I don't
have time right now. You can try same scenario with other free SIP
providers, like
fwd.pulver.com.

Not really. It should work with the same instance of ser.

As you may know translation is working fine here, but I cant connect to
users otside my domain since i get a "403 no relay" error, this is
happenig translating a prefix to an
iptel.org address.
If I dial the iptel address (number(a)iptel.or) using xlite call is
handled fine, but if i use "prefix-number" from a grandstream phone ... no way.
