You can't make 1. without a B2BUA. Who else will disrupt the call in case of zero balance?

I don't know why you need a B2BUA for either of those. On (1), you 'just' need a proxy, accounting, and some way of topping up credit (IVR and/or web site). (2) is harder. You are best-off hoping either that your media gateway will detect the lack of a media stream and close the session, or if you are proxying the media stream anyway, relying on the proxy to somehow close the session.
