As an update my collegue mistakenly executed
/usr/local/sbin/openser ul show
Note the openser, this tries to lauch openser, but since it is already
running it will not bind to any interface and kill itself, during that
process I do believe it does clean up behind it self and "correctly" it
does delete the /tmp/openser_fifo file.
So I would consider this as most errors a human error..
Am I right about this ?

Subject: [OpenSIPS-Users] Error when fetching current user list
Hi Guys
At times when I want to get the current user list using
/usr/local/sbin/openserctl ul show I get this error
ERROR: Error opening OpenSER's FIFO /tmp/openser_fifo
ERROR: Make sure you have the line 'modparam("mi_fifo",
"fifo_name", "/tmp/openser_fifo")' in your config
ERROR: and also have loaded the mi_fifo module.
I dont think this is a configuration error, as killing and restarting
openser does fix this.
