so it seems that the error happens before the transaction is created, as I
see SL in the response...
Can you try with:
https://www.kamailio.org/docs/modules/stable/modules/tm.
html#tm.f.t_set_disable_internal_reply
before t_relay() and then catch the failure execution with if(!t_relay())
and send there response you want there.
