If there is an ACK, somebody sent a 200 OK. The GW (I assume your are talking about IP-PSTN calls) should send a 200 OK only if the callee picks up the call. Maybe there is a problem with the GW or with the signaling on the PSTN side.

Some clients are complaining, why would we bill them for such calls, when there was really no one answering on the other side. What should I do to be able to identify what really are the complete calls and what are not. Thank You
