Depending on your OS the mysql configuratio is in /etc/my.cnf or /etc/mysql/my.cnf.
[mysqld] log = /var/log/mysql.log
google is your friend

Read the log files of apache/mysql/ Check if there is an incoming SQL query (enable logging in mysql configuration).
