You get stale nonces when the nonce is expired (the nonce is some kind of replay protection, it is vaild only a limited ammount of time). Have you changed the nonce_expire parameter? (by default is 5 minutes which should be enough).

Also, I am seeing 5 register requests every time, is this normal? No. Your UAs use a too low retransmission timer probably.
