each network has 128kbps, both user agent using GSM codec just to make sure (coz im not :p) is that right when both user agent use public ip, media transfered peer to peer but when one of them behind nat, media goes to server first and than transfered to other end ?

Avoid proxying RTP if both clients are on the same NAT. If the clients are running on slow residential DSL line, the bandwidth and QoS may trigger your situation because they are sharing the same uplink.
