Hello,
the PBX should support Path headers for an easy setup -- look at path
module in kamailio for more details how to use it.
If the PBX does not support Path, then you can still make it working,
but it's more complex, depending on the approach, it may involve other
modules, like uac and header operations.

Kamailio should proxy register to PBX with udp

in sip_client setting he will point to sip.mydomain.com as his server
