Has anyone had a problem like this, everything is on a public IP, even the UA, I get voice from UA --> PSTN, not the other way, the SDP all looks fine, c= has the IP address of the UA in it, I was thinking it might be a port problem, but the logs are not showing anything there.

Hi,
I don't see any detailed information but if you can do some debug on the PSTN gateway then it may helps. When debuging, check the signalling trace of TDM side (Q931 or whatever) as well.
