You can look for consultants that will do this for pay on the web site or ask on the business mailing list. In the default configuration example, I believe you have just this, but if you need help these are the resources available.

Kamailio comes with some example configs (atleast the debian packages), both kamailio-basic.cfg and kamailio-advanced.cfg contain the examples for mysql and rtpproxy. All you have to do for rtpproxy for all calls is to move the rtpproxy_manage("co") before the NAT checks.
