Well, you could increase SHMMAX, and throw more shared memory at Kamailio, I suppose. :-)

There's an interesting paper here :

Via using Intel ICC compiler instedad of gcc, they've increased the call capacity for asterisk. I think it's also operative for kamailio/rtpproxy part.
