Luca, This is a frequently asked question, there is no straight answer for it. The only recommendation I have for you is www.onsip.org, it has a very good Getting Started documents (It's not completed yet). Please take some time to browse the forum.

which part, ser going to asterisk
asterisk coming back to ser
or asterisk accepting ser
The first is just like sending any call to any other pstn/non-pstn gateway
the second is about allowing asterisk IP address into trusted in ser, so that it doesnt really ask for auth
the third is adding entries into sip.conf
