I am not sure why this (fifo_mode=0777) wasn't inserted in either the
ser.cfg or ser.cfg.sample files.

Permissions problem I'm guessing. Do you have the following in your
config file?
fifo="/tmp/ser_fifo" # named pipe needed by serctl
fifo_mode = 0777
