When the UA sends NOTIFY messages to the server it responds with "404 not found".
what is it that it is not finding. The UA's info is in the "location" table.
I am using the basic out of the box "quick start" cfg file with authentication and loacation enabled.

what's the RURI of the NOTIFY? is it intended for the server or for a user?

Are you sure you do lookup("location") also for NOTIFY requests?
Are you sure the URI in the request URI of the NOTIFY request is in the location table?
Are you using CVS head?
There is a new feature that lookup returns only contacts which are able to handle NOTIFY. (http://openser.org/docs/modules/devel/registrar.html#AEN277)
