And the B2BUA would have to keep database of user passwords very
much the same way the registrar does. This data redundancy makes
me think that such a netowrk is actually quite suboptimal -- why
can't be the registrar in the same box as proxy/nat?

fields in the message and also the gateway ser can't respond to digest
challenge from the registrar because it doesn't know user's credentials.
You will need a B2BUA for this kind of job.
