I am using STUN and that works well most of the time. I do have a problem with UA's who sit behind a symmetric firewall. ( a Microsoft server with ISA 2004 to be exact ). I understand this is a well known problem. What practical solutions can I use in combination with openser ?

Just use mediaproxy+mediaproxy module or rtpproxy+nathelper module.
To detect if a client is behind symmetric NAT usually nat_uac_test(2) is sufficient (except when there are broken clients)
