How long are you waiting from the end of dialog until checking if it’s destroyed? Default is 5 minutes after termination (typically BYE) until the dialog is destroyed

Do you have a capture of the entire call flow? If the BYE or some other message is malformed or sent at the wrong time, the dialog module may not follow the state of the dialog to hangup.
