Sounds like NAT and Firewall issues to me. Try using the NATHelper module to fix UA registration and correct NATed contacts. In particular try the * fix_nated_register()* and *fix_nated_contact()* functions.

You may also want to look into the use of "rport" and support for this on OpenSER with the *force_rport()* function.

This should fix it.
